Site Updates Auto-Updated Notifications Added

On top of push notifications which should be supported by most desktop and mobile browsers, we've now added an automatic refresh to notifications across the site.

If a tab is left open in a browser, it'll automatically poll our server for new notifications every 30 seconds and add a prefix to the page title based on the amount, ie: (1)

Some fixes today, this feature should now function a lot better - update period also updated to 12 seconds.

There's also been numerous improvements to the notification system in general, where reactions are grouped together by default, and replies aren't automatically marked as read when opening the notifications menu.
