MC and ADP claim

I hope this is OK in here.
So I have an upcoming appointment with regards to adult disability payment, renamed from PIP which I have been familiar with from years back. Should I go straight ahead and reveal my MC prescription and clinic details?
I'm unsure how this would go down with Social Security. Does anyone have experience in this area?

I sent PIP my invoice and one of my expired labels i think. Think about it, How do you work if prescribed and the work says no? So its relevant to them why be afraid you are a legit patient and it adds proof.

All is well for a few years now until they come back for re assessment ah well enjoy the peace now well done.
